SOUND HEALTH APPOINTMENT INFORMATION

The objective of this appointment is to determine which Sound frequencies might be appropriate for you based on brain dominance and reported reactions.

You will be asked to review information (via video or publications) so that you have a thorough understanding of what to expect.

Before an appointment, you will be asked to fill out client and equipment waivers.

Since our clinical opportunities are research in nature, you may be asked to help determine your body's reactions by utilizing some or all of the following biofeedback equipment; a blood pressure/pulse monitor, a pulse oximeter (measures oxygen absorption), a brain wave monitor and/or other biofeedback devices.

You have the right to ask questions regarding the procedure at any time.

Since this is an experimental technique, you or your agent must certify that you are entering into the session voluntarily and that all responsibility to follow and read the directions and materials resides with you.

This technique is not recognized by the FDA or AMA regulatory agencies.

Ordinarily insurance companies will not pay for a session.

The decision to use Sound Health Techniques or other frequency delivery systems rests solely with you.

Because of the extremely individual nature of responses to this technique, no claims can, or will, be made.

A standard Sound Health Assessment requires approximately 40 minutes to 4 hours; and can be longer for complicated or multi-faceted issues.

Using specialized equipment or computer programs, the pitch, octave and sound frequencies of your voice will be analyzed.

After identification, formulations will be constructed based on your subjective feedback and the results of objective monitoring. This feedback will be used to help determine which frequency sets are appropriate for you.

Reassessment is essential. Too much of a frequency is just as troublesome as having none at all. Outcomes may reverse if you use the sounds when they are not needed.

Clients will be asked to supply information for data research purposes.

HOW WILL IT BE DONE?

Sharry Edwards generally assesses only the most unusual cases.
We must insist that Clients be seen under these conditions:


You will be seen in the Ohio office only, unless the circumstances are extremely unusual.

Information gained will be used as a source of information for research projects and educational endeavors.

You may be asked to complete two sessions in our office. There may be a recheck the following day. This is done to ensure that you have been provided with the appropriate sounds.

You may be asked to permit an intern or student to participate in the evaluations.

You must agree to provide ongoing data during your assessment process and use of equipment.

This appointment will be used for data collection and research purposes.

Any formula set given to you must remain confidential. All formulations remain the exclusive property of Sound Health Research.

Because we require specific reports, you must agree to purchase or use an equipment set if you will be using this technology.

No perfume and cologne is to be worn during your assessment.

Arrangements for children must be made due to the fact that we cannot provide baby-sitting services.

To be eligible to borrow equipment you must be residing in the Athens area.

WHAT DO I DO NEXT?

Once your sounds have been identified:

1. You may, of course, do nothing. Just having information about your Vocal Profile could be interesting.

2. If you are so inclined, you can attempt to tone your own notes.

3. If the frequencies you need are within the musical range, you can use music.

4. The Square Two Tone Box� is a convenient, battery operated device which will allow you to have your Sound Health frequencies available to you while on the go. The Square Two Tone Box� is a frequency generator that can be set to hold many frequency combinations. Small in size, convenient, and portable, the Tone Box is limited to the sounds that have been previously set for you by your practitioner. The Square Two Tone Box™ may be used with headphones (volume control and exact specifications required) or with an amplified speaker.

AFTER YOU HAVE RECEIVED YOUR SQUARE ONE TONE BOXTM YOU WILL BE ASKED TO REPORT WEEKLY (FORMS PROVIDED) TO YOUR PRACTITIONER. WE RECOMMEND THAT YOU CONTACT YOUR PRACTITIONER WITHIN THREE DAYS OF YOUR USE OF SOUND HEALTH EQUIPMENT.

FEES ARE AS FOLLOWS:

Appointments are scheduled Tuesdays through Fridays; $125.00 per hour (2 to 4 hours normally) Mail your original contracts to P.O. Box 267, Albany, Ohio 45710, Attn: Client Coordinator. Upon approval, Sound Health will contact you to schedule your session.

Weekends, holidays, and evenings, if available, require an additional $40.00 per hour surcharge.

If you decide to use sound to address your issues, you need to purchase a Square Two Tone BoxTM. A Tone Box costs $450 (headphones included).

Use of the Sound Room: $40.00 per hour (must be scheduled in advance)

Sound Health cannot rent, make payment arrangements, or barter for services or products. Payment of services and products are expected at the time of the visit or at the end of the second day of the visit if initial session (for those who have traveled from out of town).

Third party billing is not allowed

VISA and MASTERCARD are accepted.

IF IT IS IMPOSSIBLE FOR YOU TO MEET THE ABOVE CONDITIONS, WE MAY BE ABLE TO RECOMMEND A QUALIFIED BIOACOUSTIC PRACTITIONER IN YOUR AREA.
